Position Summary:

We have an exciting opportunity to join our team as a Administrative Nurse Practitioner Coordinator in Geriatric Emergency Medicine in Manhattan. 

In this role, the successful candidate provides direct clinical care as Nurse Practitioner (NP) in the management of Emergency Department geriatric patients. Leads Geriatric Emergency Department (GED) implementation efforts and participates in care coordination of geriatric patients across care settings. Collaborates with interdisciplinary team in post discharge transitions of care including identifying appropriate primary resources to achieve optimum outcomes. Supports the leadership efforts of the geriatric emergency department in collaboration with the Senior Director of Emergency Nursing, (Advanced Practice Provider Manager), Medical Director, and core geriatric emergency department Leadership. Provides targeted assistance in the development geriatric emergency department related patient care standards, initiation and implementation of new educational programming, and expansion of geriatric emergency department services consistent with the mission and goals of NYULH.

Job Responsibilities:

I. CLINICAL
1. Provides care to older adult ED patients (65+ years in age) as appropriate. Performs complete health history, geriatric screening and assessments, physical assessments as related to geriatric emergency care, and formulates a plan of care for older ED patients.
2. Ensures seamless transition of care across settings through collaborative communication with care providers.
3. Demonstrates in-depth knowledge of assessment, nursing and medical management of stated patient populations, and learning principles as related to nursing and Advanced Practice Provider education and patient care outcomes.
4. Works collaboratively with clinical support services and other disciplines to implement and modify as necessary an appropriate plan of care that optimizes goals of care considers patient/family preferences, and that enhances quality, access, and clinical outcomes.
5. Assists the clinical staff and interdisciplinary team to evaluate patient specific plans of care based on diagnoses and interventions.
6. Prescribes medications, orders laboratory, radiological and diagnostic studies as indicated based on ED plan of care.
7. Facilitates physician/ APP/ nurse collaboration and interdisciplinary team building.
8. Functions within scope of practice and delineation of privileges as Adult or Family Nurse Practitioner.
9. Practices as a member of the allied health staff as outlined in the by-laws, rules and regulations of the Medical Staff
10. Collaborates in the design, development, and facilitation of educational offerings to all levels of clinical staff.
11. Offers learning experiences based on adult learning principles, current nursing theory, and nursing and advanced practice provider standards.)
12. Assists in the maintenance of staff education records; ensures compliance with hospital based regulatory requirements, and annual competencies.
13. Acts as a professional role model to all levels of staff.
14. Demonstrates knowledge and interpersonal skills necessary to assist staff in directing
efforts of the interdisciplinary team to meet patient needs.
15. Provides expert coaching, guidance, and clinical expertise while assisting the clinical staff with the integration of standards into practice.
16. Provides support to clinical staff in achieving certification in areas of specialty.
 

II. COORDINATION, LEADERSHIP & CARE MANAGEMENT
1. Supports the mission, philosophy, standards, goals and objectives of the institution, Departments of Nursing, and geriatric emergency department care and programs.
2. Jointly develops, implements, and maintains program related standards and program goals with the geriatric emergency department, and Nursing and APP Practice Councils.
3. Participates in developing best practices, in collaboration with the APP Manager, Sr. Director of Nursing, Medical Director, and Directors of Advanced Practice Nurses & Physician Assistants
4. In collaboration with the NYU and ED and Geriatric leadership, ensures geriatric emergency department compliance with all regulatory standards, including TJC, NYSDOH, CMS, and other related regulatory agencies.
5. Demonstrates knowledge of the organizations Core Values and incorporates them into the performance of duties.
6. Observes established NYUHC Code of Conduct at all times.
7. Conducts activities of the geriatric emergency department in a fiscally sound and effective manner.
8. Observes NYUHC compliance policies.

III. EDUCATION & DEVELOPMENT
1. Promotes own professional growth and development in clinical and leadership role.
2. Maintains current expertise in Geriatric principles, Emergency Medicine and Care Transition best practices.
3. Serves as a clinical expert and resource for the education of peers and other health professionals related to the care and special needs of geriatric patients in the ED.
4. Demonstrates strong working knowledge regarding educational program development, implementation and evaluation.
5. Collaborates with nursing leadership and advanced practice providers to plan and implement pertinent education programs for staff performance development.
6. Demonstrates effectiveness as a change agent and the ability to influence staff in the change process using alternative approaches and implementing planned change.
7. Promotes clinical scholarship among staff and the development of evidence based practice.
8. Educates patient and family to facilitate and promote healthy adjustment to change(s) in health conditions.
9. Provides geriatric specific education to community partners including EMS as needed

IV. EVALUATION OF GED OUTCOMES, QUALITY IMPROVEMENT & RESEARCH
1. Participates in the design, implementation, and evaluation of geriatric emergency department quality improvement activities and protocols.
2. Focuses on improving patient safety by evaluating policies and procedures related to current standards and guidelines in conjunction with the clinical manager.
3. Monitors patient outcomes for quality and performance improvement in conjunction with clinical leaders.
4. Uses data to inform staff, influence practice, and guide service-based quality improvement projects.
5. Works collaboratively with leadership in the development and implementation of evidenced based research into practice.
6. Actively supports service-based quality initiatives by using data to drive performance improvement.
7. Participates in service-based committees to improve patient care outcomes.
8. Participates and supports service based and institutional Quality Performance and Improvement programs and initiatives.
9. Initiates and/or participates in relevant research activities.
10. Develops performance and outcome metrics as related to GED implementation and impact.
11. Evaluates success of the program through established goals and quality metrics.
12. Recommends practice changes based on current literature, research, and evidence-based
Findings.
13. Monitors and trends the cost effectiveness and impact of the position, as it relates to efficiency and performance indicators established for GED implementation.

Minimum Qualifications:
To qualify you must have a Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York.
Education: Master’s Degree in Nursing. Certification required (as soon as feasible following graduation from NP graduate
program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical
area of practice. ACLS, PALS, NRP Provider; (Neonatal Resuscitation Program), NALS per delineation of privileges
5 years of experience as an advanced practice nurse. Evidence of ability to manage data and use of computer applications; Excellent written and verbal communication skills; Demonstrated ability to interface effectively with other disciplines and outside agencies to achieve improvement in patient outcomes, lower length of stay and efficient resource utilization. Ability to develop, implement, and sustain clinical programs; Ability to adapt to change(s); previously demonstrated ability to facilitate group processes as well as work cohesively and collaboratively as a member of the interdisciplinary team. Membership in related professional associations. Physical stamina, manual dexterity, visual and aural acuity necessary to perform all duties..

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $155,000 – $168,000 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. Schedule Interviews

Receive Offer

If you successfully complete the interview process and are identified as a finalist for the position, we will require that you complete a professional reference process. After evaluating the completed references, a decision will be made on who will receive a preliminary offer. Training & Orientation
