Position Summary:
We have an exciting opportunity to join our team as a Community Health Rep.

Together Growing Strong (TGS) is a community-based initiative within The Department of Population Health in the NYU Grossman School of Medicine. The goal of TGS is to strengthen early childhood health and wellbeing by supporting families, providers, and educators in Sunset Park, Brooklyn. The TGS team values the different cultures in Sunset Park and takes an anti-racist approach to its work. The Community Health Worker will be part of the TGS special education ambassador pilot program team. We are seeking a Community Health Rep to become the go-to person for special education among families in the Latinx community within Sunset Park. 
The Community Health Rep is a new position as a Special Education ambassador who will be an integral part of a team working to improve the way children in Sunset Park go through the special education process and transition their Individualized Education Program (IEP) from prekindergarten programs to kindergarten. Collaboratively with prekindergarten programs and elementary schools, the TGS special education ambassador pilot program aims to support families to reduce stigma, worries, and concerns about special education, guide and advocate families through the special education referral process and transition from prekindergarten to elementary school, and to be a liaison among families, schools, and evaluators/service providers. Candidates who are fluent in Spanish and English are invited to apply to this position.

Job Responsibilities:

  • Establish yourself as a trusted messenger and liaison for prekindergarten programs, elementary schools, service providers and residents in Sunset Park who are impacted by special education.
    • Become the go-to person in the Latinx community for families with children referred to special education, leading them through the process to ensure that children who would benefit from services receive them in a timely fashion. Tasks may include: (a) assisting families in preparing for special education meetings by helping them understand the timeline and steps involved, the specific special education services their child may be eligible for, and the required documentation they need to submit; (b) supporting families in understanding and processing what they are going through, and addressing any concerns or questions with kindness and empathy.
    • Become the go-to person in the community for prekindergarten programs and elementary schools making special education referrals for their families.
    • Facilitate information sessions to educate families about special education services, processes, and resources.
    • Communicate with families, programs and community providers on a daily basis, via multiple methods (in-person, phone, email)
  • Establish relationships and potential collaborations with local key stakeholders of special education in Sunset Park, such as: families of children referred to special education, schools serving the community who make special education referrals, special education service providing agencies operating in and around Sunset Park, and New York City Public Schools.
  • Collaborate with the TGS special education ambassador pilot program and formative research team.
  • Keep detailed notes on family interactions and support the TGS formative research team in identifying how we can continue to better this pilot program.
  • Participate in team meetings and supervision.
  • Complete other activities as requested. 

Minimum Qualifications:
To qualify you must have a High School Diploma and minimum of 1 year related experience in an office setting or the equivalent combination of education and experience is required.
Effective Oral, written communication and interpersonal skills.

Preferred Qualifications:
Qualifications:
o Experience working or living in Sunset Park, Brooklyn, or Latinx immigrant communities.
o Demonstrated ability or commitment to promote and advocate Special Education services in Sunset Park for the Latinx community.
o Understand the strengths, cultural norms, values, and practices in Latinx immigrant communities.
o Fluency in Spanish and English.
o Personal or professional experience with Early Intervention and/or special education.
o A detailed record-keeper who has computer knowledge and experience including in using Outlook, Excel Spreadsheets, Word, and the other means of communication and record keeping.
Preferred Qualifications:
o Strong interpersonal skills: ability to actively listen, demonstrate empathy, build rapport with others, and interact with warmth, sensitivity and tactfully.
o Ability to collaborate and work well with a wide range of people.
o Ability to work adaptively and flexibly, and multi-task.
o An excellent verbal communicator who is equally comfortable speaking with school staff, physicians, children, parents, custodial grandparents, and individuals in the community.
o Reliable and prompt the window for addressing special education needs is narrow.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $48,274.09 – $49,995.40 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

Step 1

Apply Online

The NYU Langone hiring process begins with you applying through our online portal. Be sure to update and upload your resume. Shortly after you submit your application, you will receive an email confirmation. Ten days after applying you will receive a talent assessment to be completed.

Step 2

Schedule Interviews

If selected to continue the interview process, HR will reach out via phone or email first. Then, depending on your position, they will schedule an interview with unit managers or team members. You are encouraged to dress professionally for all interviews.

Step 3

Receive Offer

If you successfully complete the interview process and are identified as a finalist for the position, we will require that you complete a professional reference process. After evaluating the completed references, a decision will be made on who will receive a preliminary offer. If you receive a preliminary offer, HR will start the onboarding process with an agreed-upon tentative start date.

Step 4

Training & Orientation

You will be contacted by an onboarding specialist who will work with you on your pre-boarding requirements. Once fully cleared, we will ask you to complete compliance orientation regulatory training. On your first day, you will attend an online required orientation to acclimate to the health system and report to your new department based on instructions provided by your hiring manager.
