The Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes nine primary care sites, 33 school-based health centers and dental clinics, four day care centers, the nation’s largest dental residency program, and New York State’s largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. The Lutheran Family Health Center (LFHC) Department of Community-Based Programs has a thirty-year history of providing educational and supportive services to community residents and their families. Our work is grounded in the philosophy that is the foundation of Lutheran’s holistic orientation to community health – that health depends upon such factors as an individual’s psychosocial, familial economic, housing educational and employment status. We are committed to affirming each family’s cultural ethnic and linguistic identities and using. Community social work practices work toward ensuring that our programs are always designed to be responsive to evolving family and community issues in the context of a diverse multicultural society. The mission of the department is to provide family strengthening services that support community residents in achieving economic and family self-sufficiency.
- Patient teaching and health education
- Assistance with patients schedules and keeping appointments with culturally and linguistically appropriate primary care and/or specialty providers
- Anticipate and help arrange for resolution of challenges to keeping appointments (e.g., transportation, childcare)
- Help patients become more proactive and address their concerns during visits with providers
- Provide patient advocacy, including interpretation, completion of paperwork, and assistance with medications
- Encourage patients participation in support groups
- Work with the healthcare team for coordination of services
- Track patient progress
To qualify you must have a Excellent communication skills including public speaking
Excellent interpersonal skills
Experience working with youth or families
Experience in volunteering and volunteer management
Foreign language speaking skills, Spanish or Mandarin
High school diploma or equivalency (or agree to pursue)
Computer literacy (PC and/or Mac), especially database and web based application
Qualified candidates must be able to effectively communicate with all levels of the organization.
