Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation’s largest dental residency program, and New York State’s largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary:
We have an exciting opportunity to join our team as a Americorps Member.

In this role, the successful candidate The Family Health Centers at NYU Langone Department of Community Based Programs has a thirty year history of providing educational and supportive services to community residents and their families Our work is grounded in the philosophy that is the foundation of the Family Health Centers holistic orientation to community health that health depends upon such factors as an individual s psychosocial familial economic housing educational and employment status We are committed to affirming each family s cultural ethnic and linguistic identities and using Community social work practices work toward ensuring that our programs are always designed to be responsive to evolving family and community issues in the context of a diverse multicultural society The mission of the department is to provide family strengthening services that support community residents in achieving economic and family self sufficiency

Job Responsibilities:

• Patient teaching and health education
• Track patient progress
• Work with the healthcare team for coordination of services
• Encourage patients participation in support groups
• Provide patient advocacy including interpretation completion of paperwork and assistance with medications
• Help patients become more proactive and address their concerns during visits with providers
• Anticipate and help arrange for resolution of challenges to keeping appointments e g transportation childcare
• Assistance with patients schedules and keeping appointments with culturally and linguistically appropriate primary care and/or specialty providers

Minimum Qualifications:
To qualify you must have a Excellent communication skills including public speaking Excellent interpersonal skills Experience working with youth or families Experience in volunteering and volunteer management Foreign language speaking skills Spanish or Mandarin High school diploma or equivalency or agree to pursue Computer literacy PC and/or Mac especially database and web based application

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $16,555.50 – $18,027.75 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
