NYC Health + Hospitals/Coler, an affiliate of NYU Grossman School of Medicine, is seeking a part time or per diem Cardiologist to provide services that will ensure top level patient/provider continuity, and ensure the coordination of service across levels of care and access to timely and appropriate care.

Job Description
POSITION OVERVIEW:
The ideal candidate will:
· Work with dedicated attending physicians, Nurse Practitioners, and ancillary staff in the Skilled Nursing Facility.
· Assist in managing the care of cardiac diseases including atherosclerotic heart disease, hyperlipidemia, heart failure, hypertension, cardiorenal syndrome, arrhythmias, valvular disease, infectious endocarditis, post cardiac surgery care and other disorders.
· Develop and monitor the delivery of plans for these conditions as required.
· Provide consulting services to other providers and scheduled off-hours telephone coverage for emergencies.
· Interpret diagnostic procedures as required and credentialled; e.g. ECG, Holter monitors, echocardiograms, ultrasound, and others.
ESSENTIAL FUNCTIONS:
· Interacts with all persons (including patients, residents, family members/visitors, workforce members, volunteers, vendors, and others) based on the organization’s ICARE values.
· Documents care and services rendered are recorded in the medical record in a timely and accurate manner.
· Collaborates with other members of interdisciplinary team in the care of patients and residents, including the family during admission, interdisciplinary meetings, and patient/family education.
· Communicates with patients, residents and their loved ones/healthcare representative(s) in an effective and compassionate manner, including change in condition and other updates.
· Communicates with workforce members, including supervisors, through team meetings/huddles to disseminate information routinely.
· Maintains professional competence through participation in continuing education and other appropriate learning experiences. Completes all mandatory training/in-services that are assigned in a timely manner.
· Performs other professional activities as delegated by the Chief Medical Officer.
· Demonstrates safety awareness and follows protocols for fire, disaster, adverse events (code management, elopement), infection control, and other safety practices.
· Assists in the orientation/training of new/novice staff through unit-based education and competency.
· Actively participates in quality assurance performance improvement activities
Minimum Qualifications
· Active NYS Medical Licensure
· Board Certified.

Step 1

The NYU Langone hiring process begins with you applying through our online portal. Be sure to update and upload your resume. Shortly after you submit your application, you will receive an email confirmation. Ten days after applying you will receive a talent assessment to be completed.
Step 2
Schedule Interviews
If selected to continue the interview process, HR will reach out via phone or email first. Then, depending on your position, they will schedule an interview with unit managers or team members. You are encouraged to dress professionally for all interviews.
Step 3
Receive Offer
If you successfully complete the interview process and are identified as a finalist for the position, we will require that you complete a professional reference process. After evaluating the completed references, a decision will be made on who will receive a preliminary offer. If you receive a preliminary offer, HR will start the onboarding process with an agreed-upon tentative start date.
Step 4
Training & Orientation
You will be contacted by an onboarding specialist who will work with you on your pre-boarding requirements. Once fully cleared, we will ask you to complete compliance orientation regulatory training. On your first day, you will attend an online required orientation to acclimate to the health system and report to your new department based on instructions provided by your hiring manager.
