Position Summary:

We have an exciting opportunity to join our team as a Patient Navigator.

 
The NYU Langone Vaccine Center is seeking a full-time Patient Navigator. In this position, the Patient Navigator will work with both the NYU Langone Vaccine Center and the LGBTQ+ Pride Center at NYC Health + Hospitals/Bellevue in the realm of Social Determinants of Health (SDoH) (i.e., housing, food, and financial issues, that affect health outcomes). The Patient Navigator will primarily work with the Pride Center staff (5 primary care physicians, 1 clinical pharmacist, 1 nurse, 1 part-time Psychiatrist, and 2 coordinators) to enhance SDOH screening and linkage to resources to address unmet needs related to SDoH. The Patient Navigator will also lead monthly meetings with community-based organizations to maintain relationships and established patient referral pathways. The Patient Navigator will also work with the Pride Center staff to foster new community partnerships to enhance LGBTQ+ care and address SDOH needs.

Job Responsibilities:

  • Completes Social Determinants of Health screeners with patients and/or research participants, facilitates referral and enrollment process with Community Based Organizations.
  • Coordinate with representatives of other related agencies/community-based organizations to communicate information and develop partnership opportunities for outreach.
  • Contributes and leads outreach campaigns through the development of innovative educational and health promotion resources to community programs.
  • Interacts with patients/staff in a courteous manner and communicates with other departments to facilitate patient flow and/or resolve patient issues.
  • Provides critical feedback to other members of the health care team based on observations in the field and outreach as necessary.
  • Complies with all organizational policies and procedures. Navigator is expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment and demonstrates confidence in decision-making and in building relationships.
  • Performs other duties as assigned in accordance with the mission, goals, and values of the NYU Langone Vaccine Center and NYC Health + Hospitals/Bellevue Pride Center.
  • Collects patient information for the research project(s). This may include abstraction of data from the patient chart (e.g., laboratory or diagnostic test results, surgical/radiation treatments delivered, adverse drug reactions, etc); abstraction of data for publications, or data collection from outside physicians offices. Uses tools to facilitate data collection (e.g. calendars, schedules, tracking logs, etc).
  • Audits and updates the database or the case report forms. Conducts study visits, obtains and documents information within the time frame specified. Research, collects, compiles and conducts preliminary analyses of data, statistics, and other materials for reports. 
  • Conduct preliminary analysis.
  • Recruitment – Screens potential patients/subjects for eligibility to the study and schedules the initial visits. This may include researching and gathering information from the medical record, physician referral, advertisement and directly scheduling a visit to evaluate the patient/subject. 
  • Reviews all the elements of the screening process with the Principal Investigator that include but not limited to: inclusion/exclusion criteria, completed informed consent, documentation of the event and the patient/subject willingness to participate

Minimum Qualifications:
To qualify you must have a Bachelor’s degree and/or equivalent combination of education and experience; one year experience in case management/coordination; specifically supporting members of the transgender and LGBQ communities.

Preferred Qualifications:
Bilingual (English/Spanish) preferred. Previous experience working with the LGBTQ+ community. Knowledge of basic medical terminology.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $64,350.00 – $85,407.34 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

Step 1

Step 2

Schedule Interviews

If selected to continue the interview process, HR will reach out via phone or email first. Then, depending on your position, they will schedule an interview with unit managers or team members. You are encouraged to dress professionally for all interviews.

Step 3

Receive Offer

If you successfully complete the interview process and are identified as a finalist for the position, we will require that you complete a professional reference process. After evaluating the completed references, a decision will be made on who will receive a preliminary offer. If you receive a preliminary offer, HR will start the onboarding process with an agreed-upon tentative start date.

Step 4

Training & Orientation

You will be contacted by an onboarding specialist who will work with you on your pre-boarding requirements. Once fully cleared, we will ask you to complete compliance orientation regulatory training. On your first day, you will attend an online required orientation to acclimate to the health system and report to your new department based on instructions provided by your hiring manager.
