NYU Langone Hospital—Suffolk is a 306-bed medical center, providing care to residents of eastern Long Island. The hospital facility is home to the Knapp Cardiac Care Center—an advanced heart disease diagnostic and treatment facility—as well as a modern ambulatory surgical pavilion with specialized services including women’s imaging, a sleep laboratory, and bariatric surgery. Our Provisional Level 2 Trauma Center delivers comprehensive and specialized care for patients with traumatic injuries. The Stroke Center at NYU Langone Hospital—Suffolk is designated by the New York State Department of Health as a Primary Stroke Center, with expert neurologists available to provide treatment 24 hours a day, 7 days a week. Additionally, NYU Langone Hospital—Suffolk has multiple outpatient sites, including an outpatient wound care center, a hemodialysis center, and various primary care and specialty offices.

Position Summary:
We have an exciting opportunity to join our team as a Clinical Pharmacologist – Pharmacy – Full Time – Pharmacy – – Position requires a flexible work schedule. 

In this role, the successful candidate The Clinical Pharmacologist will provide safe, efficacious and cost-effective pharmacotherapy to patients for assigned clinical services with a focus on Transitions of Care. Is responsible for helping to establish correct medication history at hospital admission, medication reconciliation and education. Duties include obtaining Best Possible Medication History (BPMH), appropriately documenting medication histories in our health systems Electronic Health Record (Epic), and collaborating with the pharmacy team and other healthcare providers as needed. Additional duties include Updating Prior to Admission (PTA) Medication list, facilitating changes in medication therapy and Documenting changes in EPIC

Job Responsibilities:

  1. Clinical Skills

Performance Expectations:

    Monitors the use of medication in relation to the patients age, height, weight, sex and disease state to assure appropriate utilization.
    Interfaces with the medical staff to suggest therapeutic alternatives that are both cost effective and do not compromise the level of care.
    Collects data on existing medical practice at the facility and compares it to data in the literature to make recommendations for improvement of medical and pharmaceutical care
    Evaluates the pharmaceutical and medical literature for pertinence at the facility and for individual patients.
    After evaluation of the clinical evidence, works with the Medical Staff to make recommendations for changes or non-changes to the Hospital Drug Formulary as new treatments become available
    Develops and amends policies and procedures relating to Pharmacy Services transitions of care.
    Participates in rounds with residents to promote appropriate drug usage, education and cost containment.
    Acts as an educational resource by developing and presenting programs to the Pharmacy and hospital staffs and patients that promote the safe and effective use of medications.
    Works with the Medical Staff to present, promote and educate the Medical Staff.
    Develop rational drug therapy, assessment of treatment, and monitoring drug therapy.
    Maintains a clinical pharmacy practice at the facility and performs consults on individual in-patients and outpatients when requested by the attending physician.
    Makes progress report and educational presentations at appropriate committees regarding Pharmacy Services transitions of care.
    Development, with the Medical Staff and Administration, plans for continued growth of the Pharmacy Services transitions of care program

    Develops activities aimed at containing the drug budget while maintaining a high standard of care.
    Develops and amends policies and procedures relating to the Pharmacy Services transitions of care
    Acts as preceptor for pharmacy students and residents
    Manage pharmacy staff involved in transitions of care
    Provides input on recruitment by conducting interviews, hiring, orientating, training, and evaluating the performance of subordinate personnel.
    Provides discipline to subordinate personnel, when necessary.
    Maintains knowledge of current trends and developments in the field of clinical pharmacy by reading appropriate books, journals, related literature, attending seminars and conference, etc.
    Develops and formulates individual projects 
 

  1. Administrative Skills

    Directs activities of the pharmacy staff involved in transitions of care.
    Develops activities aimed at containing the drug budget while maintaining a high standard of care.
    Prepares reports relating to Pharmacy Services transitions of care 
    Determines the value of drug therapy to relate medication expense to improved patient outcome
    Performs preparation and presentation of information needed related to Pharmacy Services transitions of care
    Supervises all medication use studies and monitoring activities performed by Pharmacy Services transitions of care
    Manages the Pharmacy Services transitions of care program
    Formulates plans for future activities of the Pharmacy Services transitions of care

Minimum Qualifications:
To qualify you must have a Doctor of Pharmacy degree from an accredited school of pharmacy
NYS Pharmacy License and Registration
Self motivation, excellent communication, organizational, and interpersonal skills..

Required Licenses: Pharmacist License-NYS

Preferred Qualifications:
Experience in clinical and managerial areas preferred
Post-Graduate Year-1 Pharmacy Residency or Board Certification in Medication Therapy Management or appropriate specialty is strongly preferred

Schedule Interviews

Step 3

Receive Offer

If you successfully complete the interview process and are identified as a finalist for the position, we will require that you complete a professional reference process. After evaluating the completed references, a decision will be made on who will receive a preliminary offer. If you receive a preliminary offer, HR will start the onboarding process with an agreed-upon tentative start date.

Step 4

Training & Orientation

You will be contacted by an onboarding specialist who will work with you on your pre-boarding requirements. Once fully cleared, we will ask you to complete compliance orientation regulatory training. On your first day, you will attend an online required orientation to acclimate to the health system and report to your new department based on instructions provided by your hiring manager.
