Position Summary:
We have an exciting opportunity to join our team as a Telephonic Care Nurse.
NYU Langone Department of Medicine Division of Geriatric Medicine and Palliative Care presents an exciting opportunity for nurses passionate about delivering telephonic palliative care management In this role the successful candidate will provide palliative care consultations to Patients Living with Alzheimers Disease and other related dementia after discharge from the Emergency Department or Emergency Department Observation Unit Nurse led telephonic case management will be conducted centrally for 82 participating clinical sites across the country. NYU Langone Health Key aspects of palliative care discussions will include establishing goals of care exploring end of life wishes discussing advance care planning with healthcare providers and addressing pain and symptom management The successful candidate will receive proper training on telephonic administered care to ensure appropriate patient clinician communication and effective palliative care delivery The nurse will work under the supervision of a board certified palliative care physician

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $43.39 – $55.00 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
