Position Summary:
We have an exciting opportunity to join our team as a Representative, Health Information Access – Mineola.
In this role, the successful candidate is responsible for facilitating the handling of PHI Protected Health Information requests in accordance with Federal and State statutes referencing release of information as well as Hospital and department policies and procedures received by the Correspondence Unit of HIM Department and via phone or in person by patient or patient representative. Process paper and scanned medical records in a hybrid environment using multiple systems to access information and distribute requested PHI in hardcopy and electronic formats. Provide requested information to other healthcare providers and review and accept HIPAA Authorizations for Patient Information Bill copy and maintain Log of daily activities. Handle and process Subpoenas and Patient Access requests and provide records to IPRO RAC 3rd party payers and other financial audits as requested by the Utilization Review Dept and Finance Department. Act as a liaison to the Risk Management Department.
Job Responsibilities:

- Provide accurate information to patient or patient representative and evaluate for compliant HIPAA authorization paying keen attention that ID is checked and completed on authorization Processes information timely
- Process daily incoming mail for requested information from patients attorneys insurance companies healthcare agencies and other providers or requestors Track mail to secure the correct identification of the patient and their medical record number and ensure proper authorization is received Correctly identified patient MRN Medical Record Number in Computer system and review records for completeness accuracy and compliance with regulations
- Provide HIPAA authorization via email or fax to expedite the Emergency and STAT requests for records Provide requests to Healthcare providers for ongoing patient care and manage all requests by accurately logging into CHARTRELEASE Consistently protect the privacy of medical records to ensure confidentiality is maintained
- Accurately log the daily mail into CHARTRELEASE management application for the ongoing monitoring of all correspondence Record complete and accurate information in CHARTRELEASE to include all dates of service and any special notes or instructions Distribute mail into correct folders for proper handling
- Pull charts for copying and scanning records in a hybrid record environment Using multiple systems access accurate information to provide PHI to requestor Provide records on CDs copies from Microfilm and other electronic formats Properly track charts to the next chart location and coordinate invoices for all billable accounts
- Perform review and quality check on all outgoing mail Properly log out the completed copies of records in CHARTRELEASE for management of all requests Scan all HIPAA requests into Legal Correspondence Tab of the scanned record for storage and coordinate the retrievals and returns of records from the remote storage companies
- Process all Patient Access Requests by accepting and processing Subpoena s for medical records within the required 10 days Review and accept Subpoenas for proper HIPAA authorization Secure radiology information and notify patient s accounts as needed
- Act as a liaison to the Risk Management Department Coordinate the Legal File charts pulls and tracks as needed for proper locations Act as the point of contact for servicing the copier fax or microfilm as needed
- Perform other duties or special projects as assigned by the Manager or other designated person
Minimum Qualifications:
To qualify you must have a High School or GED. 1-2 years General office experience working in a Healthcare environment. Basic computer skills specifically, Microsoft Office and knowledge of email. Strong communication skills both written and verbal. Strong attention to detail. Strong customer service skills with ability to interact with both internal and external customers. Job requires carrying lifting pushing bending as well as other physical duties.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $53,101.10 – $55,621.74 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
