Position Summary:
We have an exciting opportunity to join our team as a Physical Therapist, Per Visit – Home Health Care – Eastern Long Island.
In this role, the successful candidate provides safe therapeutic age appropriate physical therapy to promote the improvement of functional mobility and physical ability and quality of life. Uses clinical expertise to plan the delivery of care through examination evaluation planning implementation and re assessment

Minimum Qualifications:
To qualify you must have a Bachelor’s Degree Physical Therapy NYS Registered or APTA Accredited Physical Therapy Program Current and valid NYS Physical Therapy license and registration 1- 2 years Hospital Inpatient or Orthopedic Outpatient or Pediatric setting Excellent communication both oral and written critical thinking and customer service skills Basic computer skills
Preferred Qualifications:
Master’s Degree Physical Therapy NYS Registered or APTA Accredited Physical Therapy Program Post Doctoral Doctorate in Physical Therapy DPT Physical Therapy NYS Registered or APTA Accredited Physical Therapy Program, 1 – 2 years Home Care experience
